Cardiology Question for the Week of July 6, 2020

Question:

How is the following scenario coded? A patient undergoes PTCA and bare-metal stenting of the LC OM1 via a SVG and an additional PTCA and bare metal stent of an OM2 lesion via a completely separate SVG.

Answer:

Since two different ostia are cannulated, code 92937 (PTCA, single vessel) is used for therapy of the OM1 branch and 92937 (PTCA) for angioplasty of the OM2 branch, because it was treated via a separate bypass graft.

92937 Percutaneous transluminal revascularization of or through coronary artery bypass graft (internal mammary, free arterial, venous), any combination of intracoronary stent, atherectomy and angioplasty, including distal protection when performed; single vessel
