Question:
What are the key 92941 reporting requirements for PCI performed to treat thrombotic coronary lesions associated with an acute myocardial infarction?
Answer:
PCI of thrombotic coronary lesions performed nonemergently after recent myocardial infarction. Additionally, the AMA clarified that it is not necessary to perform more than one type of intervention (i.e., PTCA and stent, atherectomy and stent, atherectomy and PTCA) listed in the code descriptor of the AMI code in order to assign 92941. As the AMA states, “Note that code 92941includes any combination of intracoronary stent, atherectomy and angioplasty, including aspiration thrombectomy when performed. Code 92941 is, therefore, appropriately reported if any one or more of these services is performed to treat the lesion.” Remember: If a patient who has a myocardial infarction is stabilized, and then one or two days later is electively brought to the catheterization lab for a procedure, based upon the information provided above, this would not meet the requirements for a procedure performed during an AMI. (That is, do not assign 92941 for this service in this scenario.) In the description of code 92941, the AMA clarifies the term “subtotal occlusion” to mean there is markedly reduced flow through the lesion. “There is no number assigned here. The subtotal occlusion must be present in the setting of an acute myocardial infarction (AMI). A chronic subtotal occlusion in a patient who does not experience acute MI would not meet the criteria for this code.”
