We prepared a patient dose from a single-dose vial containing 100 mg, and the HCPCS descriptor was 100 mg. The administered dose was 75 mg, so the discarded waste was 25 mg. What would be the billing unit?

Answer:

The total of the administered plus discarded waste is equal to the HCPCS billed unit of one. Billing another unit of service on a separate line item with a HCPCS code and modifier JW appended for the discarded 25 mg of drug is not permitted because it would result in overpayment.
