Is J9002 still the correct code to bill for doxorubicin HCL?

Answer:

Effective July 1, 2016, the Centers for Medicare & Medicaid Services (CMS) directed its contractors (via transmittal 1630OTN, issued February 26, 2016) to replace expired level II J9002 (injection, doxorubicin HCL, liposomal, doxil, 10 mg) with the following as a covered HCPCS code and revise all related edits effective for claims with dates of service (DOS) on and after October 1, 2015.

Q2050 Injection, doxorubicin hydrochloride, liposomal, not otherwise specified, 10 mg
