Answer:

Code 76016 describes a written safety determination report from the physician or other qualified healthcare professional (QHP) responsible for the performance of the MR procedure. This is done when the implant/foreign body contains insufficient conditional labeling, when MR is contraindicated, or when a limited MR procedure may be necessary.
