Question:

Would we report modifier -26 for a Swan-Ganz insertion on a professional claim?

Answer:

Professional claims do not need to report modifier -26 for a Swan-Ganz insertion. However, if a Swan-Ganz is used for a right heart catheterization reported with code 93451, modifier -26 should be reported for a professional claim.
