Answer:

Codes 36465 and 36466 are reported for injection of a non-compounded foam sclerosant into one or more extremity truncal veins such as the great saphenous vein. Report 36465 when one vein is treated in a leg, and 36466 when more than one vein is treated in the same leg. “Non-compounded” means that the drug has not been combined, mixed, or altered to treat the patient. When a compounded foam sclerosant is used, report 36470 or 36471 instead of 36465 or 36466. Codes 36465 and 36466 include ultrasound compression maneuvers. If the injection is performed without compression maneuvers, report 36470 or 36471. A common name to help you select the correct codes that you may see regarding codes 36465–36466 is “Varithena” or “1% polidocanol.”
