When is a one-time HCV screening test covered for adults who do not meet the high risk definition?
A single, one-time HCV screening test is covered for adults who do not meet the high risk definition, but who were born between 1945 through 1965. This claim must be submitted with HCPCS G0472. Based on the information in the Preventive Benefit manual, this coverage is age dependent based on date of birth.
