Question:

When coding for a diagnostic bone marrow biopsy and aspiration, how should you report the aspiration if it is performed through the same incision as the biopsy, and what codes should be used if the aspiration is performed from a separate site? Would we use G0364?

Answer:

HCPCS code G0364 for reporting of the aspiration in addition to the biopsy through the same incision was deleted and replaced with CPT® code 38222 (Diagnostic bone marrow; biopsy[ies] and aspiration[s]). When it is necessary to report an aspiration from a separate site— such as aspiration and biopsy left iliac crest with aspiration right iliac crest—report 38220 with an appropriate modifier in addition to 38221 or 38222.
