What are the reporting differences between new 2026 code 99445 and the 2026 revised code 99454?
New code 99445 is for 2-15 days in a 30-day period; revised code 99454 is for 16-30 days. These codes are mutually exclusive and should not be reported together during the same 30-day timeframe.
