Analyzing ASC C Cardiology Coding for 2023

Coders are facing critical challenges in 2023, with many scenarios at stake that could upend success and ultimately leave reimbursement vulnerable. Among the codes that impacted cardiology this year are the series of over fifty new C codes (C7500 – C7555) for Ambulatory Surgical Centers (ASCs). Even though these are not relevant to hospital facility claims/OPPS, they still impact many procedures including Cardiology, IR, and EP. Let’s review the nuances for accurate understanding in 2023.

Medicare Billing Caution

Understand that these codes are not applicable to facilities that code under the OPPS. The series has received a designation of status indicator of E1. E1 designates that items, codes, and services are not covered by any Medicare outpatient benefit category; are statutorily excluded; and are not reasonable and necessary. In addition, they are not paid by Medicare when submitted on outpatient claims or any outpatient bill type. Understand that hospital facility coding should not use these HCPCS codes. 

Coding Specificity

C7500–C7555 are exclusively designed for ASCs, aiming to provide supplementary payment for packaged services. These codes allow additional payment for packaged services for ASCs, and in certain scenarios, a combination of C codes and Category 1 Codes will be essential for accurate reporting. It’s crucial to note that the code descriptors are highly specific to particular sets of services. Additionally, a key consideration is the requirement of Modifier 52 if a Fractional Flow Reserve (FFR) procedure does not include pharmacological stress on the heart.

Understand that this coding set is extremely specific to diagnostic coronary angiograms / coronary with L heart / coronary with R heart / coronary with R&L heart /coronary with grafts / coronary with grafts and R heart/ coronary with grafts and L heart/ coronary with graft and R&L heart/ revascularization on some body parts that are performed with either:

  • IVUS
  • FFR
  • Non-selective abdominal/lower extremities (with coronary). Here, coders must be cautious that these additional services are presently under OPPS/ an add-on code, and not separately payable. Creating new primary codes for ASCs has provided a way for additional payment.

Be aware that these codes will need to be reported with other PCI codes when performed. Note too that it is necessary to check modifier application for performing a diagnostic study with a PCI. The following codes are those from the series that impact cardiology services specifically:

C7517Catheter placement in coronary artery(s) for coronary angiography, including intraprocedural injection(s) for coronary angiography, with iliac and/or femoral artery angiography, non-selective, bilateral or ipsilateral to catheter insertion, performed at the same time as cardiac catheterization and/or coronary angiography, includes positioning or placement of the catheter in the distal aorta or ipsilateral femoral or iliac artery, injection of dye, production of permanent images, and radiologic supervision and interpretation
