When the patient is scheduled for a right ventricular lead insertion on her pacemaker, is CPT® code 33216 the only code for which I would need authorization? The diagnosis is right ventricular lead failure.

Answer:

Yes, the correct CPT code is 33216 (insertion of a single transvenous electrode, permanent pacemaker or implantable defibrillator). You should also get pre-authorization for codes 33234 or 33235 if the defective lead is removed.

33234 Removal of transvenous pacemaker electrode(s); single lead system, atrial or ventricular
33235 dual lead system
