Question:

When do we report codes 93016 and 93018 as opposed to 93015?

Answer:

Codes 93016 and 93018 are the component codes of 93015. They should only be used separately to identify the components of service that the physician performs if the physician does not own the equipment or employ the personnel, when the service is provided in a hospital-based setting. Medicare
does not accept code 93015 for the professional component of this service in a setting where the
physician does not own the equipment.
