Question:

Do we need to report start and stop times in the medical record when reporting 0540T?

Answer:

Understand that Code 0540T is not a time-based service and therefore reporting of the code does not rely on the start and stop time documented in the medical record. However, documentation of the initiation of the cell therapy and discontinuance will be commonly documented since clinicians will be carefully monitoring for possible side effects.
