Can you tell me more about the requirements for the Z-Code?

Answer:

The Z-code is not required for hospitals in non-MolDX states. They are not required for inpatient claims and are not required for payers other than Medicare but you may see the MGD Medicare or Medicare Advantage payers request a Z-code be added to the claim when billing for outpatient procedures.
