Radiology Question for the Week of January 20, 2020

Question:

Do you have any guidance for reporting CT of the spine with myelography codes?

Answer:

Do not report CT of the spine with intrathecal contrast with myelography codes 72240–72270 unless both studies are medically reasonable and necessary. Often radiologists will call an intrathecal injection under fluoroscopic guidance a CT myelogram. This should not be reported with the myelography codes. Myelography requires documentation of a formal study with written interpretation and report. Radiography after injection of intrathecal contrast to perform a CT of the spine in order to confirm the location of the contrast is not separately reportable as myelography
