Answer:

As you appear to know, 94729 is an add-on code, which is why there is a + sign in front of the code. The answer to your question can be found by reviewing the parenthetical notes in the CPT manual, which indicate the following codes: 94010, 94060, 94070, 94375, 94726, 94727 or 94728
