For codes G0237 and G0238, what are the documentation requirements for their use?
The therapist’s notes should provide clear evidence of the patient’s progress, or lack of progress, toward reaching the goals as established by the physician. (If, at any time during the patient’s participation in the program, it becomes apparent that the anticipated goals will not be met, the services will no longer be considered reasonable and necessary.)
