What are the requirements for using 96376 with 96374?

Answer:

Code 96376 may be reported with either 96374 or 96375. Two criteria must be met when applying this code: at least 31 minutes must have passed since the last IV push of the same drug, and the additional administration must be a separate preparation of the drug.

96374 Therapeutic, prophylactic, or diagnostic injection (specify substance or drug); intravenous push, single or initial substance/drug
96376 Therapeutic, prophylactic, or diagnostic injection (specify substance or drug); each additional sequential intravenous push of the same substance/drug provided in a facility (List separately in addition to code for primary procedure)
