Which revenue codes should be used to report vaccines and their administration to Medicare?

You can find the answer to this question in the Medicare Claims Processing Manual, Chapter 18—Preventive and Screening Services, section 10.2.2 at https://www.cms.gov/Regulations-and-Guidance/Guidance/Manuals/downloads/clm104c18.pdf. The information in this section applies to all providers except rural health clinics (RHCs) and federally qualified health centers (FQHCs); information for billing influenza virus, pneumococcal and hepatitis B vaccines in RHCs and FQHCs can be found in section 10.2.2.2 of that chapter.
