Yes, this code includes fluoroscopic and CT guidance. Do not report code 64451 with codes 64493, 64494, or 64495. When performed bilaterally assign modifier -50 to code 64451. Use this code for a block, not a destruction Do not report with codes +95873 (Electrical stimulation for guidance in conjunction with chemodenervation) or +95874 (needle electromyography for guidance in conjunction with chemodenervation).
