Question:

It is appropriate to report code 76098 twice if two separate interpretative encounters occur?

Answer:

If two separate specimens from the same lesion are separately submitted and interpreted at two separate interpretive encounters, it is appropriate to report code 76098 twice. If multiple specimens of the same lesion are simultaneously submitted without reference to time, code 76098 should be reported only once.
